Using Care4AU as a support coordinator

Care4AU is a growing Australian directory for disability, aged care, allied health and home support. Support coordinators can browse provider profiles and, with a participant’s permission, submit a free request to begin an enquiry. Care4AU does not provide support coordination, make referrals on a participant’s behalf without consent, or decide whether a support can be funded.

Before you submit a request

Confirm that the participant wants to explore providers, what service they are seeking, what location and timing matter, and who may receive their information. Use plain language and include preferences that affect a good fit. The form asks for an email address so Care4AU can reply; it does not require a phone number.

Share only what is needed

For an initial enquiry, avoid NDIS numbers, complete plans, Medicare details, bank information, passwords and detailed clinical records. If a provider later needs additional information, the participant can decide what to share and through which secure process. Care4AU asks before passing the request to a provider.

Help the participant compare

Encourage questions about service scope, location, capacity, timing, fees, travel, cancellation, qualifications, registration where relevant, agreements, complaints and backup arrangements. Verify critical details directly. Directory profiles may be incomplete or out of date; a claimed listing is not an endorsement or quality audit.

What to expect from Care4AU

We may contact you by email to clarify a request. We cannot guarantee a provider response, current availability, a match or funding. The participant remains free to proceed, request more information or stop the enquiry. Requests are free during early access and do not create a commitment to use any provider.

Read the referral checklist, participant request process and privacy information. Use the free request form or contact Care4AU using the website form.
