How participant support requests work

Care4AU gives participants, family members and support coordinators a free way to start an enquiry about disability, aged care, allied health or home support. The request form is not an application for government funding and does not commit anyone to a provider.

Step 1: Tell us the basics

Choose the type of support, location and preferred timing. Describe the need in everyday language. Share preferences that could help a provider understand the request, such as language, accessibility or scheduling. An email address is required so Care4AU can reply; a phone number is not required.

Step 2: We review the request

Care4AU reviews the details and may ask for clarification by email. Please include only information needed for an initial enquiry. Do not send NDIS or Medicare numbers, bank details, passwords, full plan documents or detailed health records through the form.

Step 3: You decide whether to share

We ask before sharing the request with a provider. If you agree, the provider may contact you using the information you authorised. You can ask questions, request changes or decide not to proceed. A provider might not respond or have capacity, and a directory listing does not confirm eligibility, suitability, registration or funding.

Step 4: Compare and choose at your own pace

Ask about availability, service scope, who will provide support, fees, travel and cancellation costs, written agreements, complaints and continuity. Check registration in an official register when relevant. Verify plan rules and aged-care eligibility with the responsible government service. You remain in control of the choice.

Who may submit a request?

A participant may submit their own request. A family member, carer or support coordinator can help when they have the participant’s permission and accurately represent their preferences. If you are submitting for someone else, explain the process and confirm what they want shared.

What does it cost?

There is no charge for participants or coordinators to submit a request. Care4AU does not charge for a provider introduction through this form during early access. Any future changes will be explained clearly before they take effect.

Start with the free support request form, read our privacy information or use the contact form if you have a question. If someone is in immediate danger, call 000; Care4AU is not an emergency service.
