Help for NDIS support coordinators

Care4AU is an independent Australian directory for disability, aged care, allied health and home support. Support coordinators can use the directory to explore providers or send a free enquiry with a participant’s permission. Care4AU does not manage plans, recommend a provider, verify individual suitability or make funding decisions.

Use the directory as a starting point

Browse by service and location, then confirm key details directly with providers. A listing may be incomplete or out of date, and the directory is still growing. Check availability, service scope, costs and registration where relevant. Do not assume a profile means the provider is appropriate for a participant’s funding arrangement.

Submit a participant request with consent

Before using the form, confirm the participant wants help and agrees to what will be shared. Include only the basics: service type, suburb or region, timing, relevant funding context if known, and communication or accessibility preferences. Care4AU uses email to respond; the form does not ask for a phone number. We ask before sharing details with a provider.

Do not include a full plan, NDIS number, Medicare number, bank details, passwords or detailed health history in the open form. Share sensitive information later only when necessary, with permission and through an appropriate secure channel.

Information to collect from providers

Ask whether they serve the location, have capacity, accept the relevant arrangement, and can meet the participant’s preferences. Request a written explanation of service scope, fees, travel and cancellation charges, worker qualifications or registration where relevant, service agreement, complaint pathway and continuity arrangements.

Keep participant choice central

Present options clearly and in an accessible format. Explain differences in availability, cost and service approach without pressure. Give the participant time to ask questions and choose whether to proceed. Document consent, referrals and responses according to your professional obligations and organisational processes.

What Care4AU can and cannot do

We can receive an enquiry, ask for clarification by email and, after permission, share it with a provider. We cannot guarantee a provider response, a match, current capacity, funding, quality or a particular outcome. Use formal provider and regulator channels for service complaints or safety issues; call 000 if someone is in immediate danger.

Use our referral checklist, read how requests work and privacy information, or start a free support request. Check current NDIA information on working with a support coordinator.
