For a support request, we ask for your name, email address, location and the type of support you are looking for. Funding, timing and practical preferences are optional. For a provider profile, information may include business details and business contact details. A provider may choose to publish a business phone number, email address or website.

Use the forms safely

Please do not enter NDIS numbers, dates of birth, diagnoses, medical reports or detailed health information in Care4AU forms. Share only what is needed to describe the service you are seeking. You can discuss sensitive details directly with a provider after you decide to contact them.

How requests are handled

Care4AU uses your request to understand what help you are seeking and reply by email. We will ask before sharing any request details with a provider. Provider profile information is used to run the directory, review or update a listing, and contact the business about its profile. Care4AU does not promise a match or a provider response.

Services that process information

Information submitted through this website may be processed by the website host and services used to operate forms, prevent spam, measure site use and send email. Those services handle information under their own privacy terms. Use the Care4AU contact form if you want to ask about a particular submission.

Access, corrections and removal requests

You can ask us to provide a copy of information you submitted, correct it or delete it using the Care4AU contact form. Tell us what the request relates to and, where possible, use the email address you submitted with it. We may need to keep some records where the law requires it or to resolve a genuine service or security issue.

How long we keep information

We keep information only for as long as it is reasonably needed to respond, operate the directory, maintain appropriate records or meet legal obligations. You can contact us to ask about a particular submission or request its removal.

Keeping information safe

We take reasonable steps to protect information handled through Care4AU. No website or email service can guarantee that information sent over the internet is completely secure, so please do not submit sensitive personal or health information through these forms.
