Planning for an NDIS plan reassessment or review
Preparing for an NDIS reassessment or plan review can help a participant explain what has changed and what supports they need to pursue their goals. The NDIA makes decisions about plans and funding. Care4AU cannot advise whether a support will be funded or determine a person’s eligibility.
Start with current goals and daily needs
Ask the participant what is important to them now, what they want to work towards, and what has changed since the last planning conversation. Note practical examples from everyday life, including what is going well, where support is difficult to access and what the person wants to do differently. Use the participant’s own words and preferred communication method.
Gather relevant information
Check the NDIA’s current instructions for the review type and documents requested. The participant may choose to gather recent assessments, reports, service information or other evidence relevant to their circumstances. Keep copies and only share information with people who need it and have permission to receive it. Do not assume every document is required; ask the NDIA or a qualified adviser if uncertain.
Describe supports clearly
For each support being discussed, explain the functional need, the goal it relates to and the practical impact. If a current arrangement is not working, describe why and what alternative approach the participant wants to explore. A provider quote or service description may help explain a proposed support, but it does not guarantee NDIA funding.
Prepare questions and communication preferences
- How will the review take place, and what accessibility or communication supports can be arranged?
- What information should be provided, and by when?
- Who may participate and receive information, with the participant’s consent?
- What should the participant do if their circumstances or support needs change before the review?
- How can the participant request clarification or raise a concern about the process?
After the review
Read any plan or decision carefully and ask the NDIA to explain anything unclear. Confirm dates, budgets, management types and applicable conditions from official documents. If the participant disagrees with a decision, check the current review rights and time limits directly with the NDIA or an independent qualified adviser.
Choosing providers remains the participant’s decision
Funding approval and provider suitability are separate questions. Check a provider’s availability, costs, service agreement, qualifications and registration status where relevant. Ask about cancellation, travel and continuity before support begins.
